How to refer
- Fax the referral to (832) 916-2480: your order or our form, patient demographics and insurance, and the reason for referral.
- Attach recent notes, imaging reports (MRI, CT, X-ray) and any prior injection, surgery or therapy records. Images on disc are not required — reports are enough to start.
- For urgent cases, call (832) 916-2075 and ask for the referral coordinator so the patient can be worked in.
- We call the patient within one business day, verify benefits and authorization, and schedule at the office they prefer.

Who we can help
Interventional evaluation and treatment for spine, joint and nerve pain, including:
- Low back pain, neck pain, sciatica and cervical radiculopathy
- Disc herniation, spinal stenosis, facet and SI joint pain
- Vertebral compression fractures (kyphoplasty candidates)
- Persistent pain after spine surgery, CRPS, neuropathy and other nerve pain
- Knee, hip and shoulder pain, including pain after joint replacement
- Work and auto injuries, postherpetic neuralgia, post-amputation pain and cancer-related pain
Not a fit: primary headache disorders (we do offer occipital nerve blocks on neurology referral), acute trauma needing imaging or admission, and patients who require inpatient care.
What happens after you refer
- Evaluation by a board-certified, fellowship-trained pain physician; the patient’s imaging is reviewed at the visit.
- Diagnostic blocks when the pain generator is unclear, before any longer-lasting procedure.
- A written plan the patient and you can follow: procedures, therapy, medications or referral back to you or to a surgeon.
- Consult note faxed to you after the first visit; procedure notes after each intervention; a call from the physician for anything urgent.

Frequently asked questions
Do you require a referral?
Most PPO and Medicare patients can self-schedule. HMO plans and workers’ compensation carriers usually require a referral or authorization; our team checks each plan before scheduling.
How quickly will my patient be seen?
We contact the patient within one business day of receiving the referral. Most new patients are scheduled within one to two weeks; call and ask for an expedited visit when a patient has severe or progressive symptoms.
Do you manage long-term opioid therapy for referred patients?
We evaluate every patient individually. Our focus is interventional and multimodal care; medication management is offered under a treatment agreement with monitoring, and we coordinate closely with the referring physician. See medication management.
Which patients are not a good fit?
Primary headache disorders such as migraine (neurology), acute fractures or infections needing hospital care, and surgical emergencies. We do not currently accept Medicaid. When we cannot help, we say so and suggest where to send the patient.
Will I receive notes?
Yes. A consult note is sent to the referring provider after the first visit, and procedure notes follow each intervention. Tell us if you prefer fax or a specific EHR direct address.
