What this policy covers
This policy describes how the Gulf Coast Pain & Spine website (gulfcoastpain.com) collects and uses information. It does not replace our HIPAA Notice of Privacy Practices, which governs your medical information and is provided at your first visit; request a copy at any office or by calling (832) 916-2075.
Information we collect on the website
- Information you provide when you request an appointment through our scheduling partner (NexHealth) or contact us: name, contact details, insurance and the reason for your visit. Appointment requests are transmitted to and stored by our scheduling vendor under a business-associate agreement.
- Usage information collected automatically: pages visited, approximate location, device and browser type, and referring site, through standard server logs and analytics cookies (Google Analytics).
- We do not sell personal information and do not use advertising trackers that share health-related browsing data with third parties.
How we use it
- To respond to appointment requests and questions.
- To operate, secure and improve the website.
- To measure which pages are useful and where visitors come from, in aggregate.
Email and portal
Please do not send medical information through unsecured email. Use the patient portal (opens in new tab) for records, results and messages, or call the office.
Your rights
Under HIPAA you have the right to access and receive a copy of your medical record, request corrections, request restrictions, receive an accounting of certain disclosures, and file a complaint with the practice or with the U.S. Department of Health and Human Services without retaliation.
