Insurance and referrals
What to know before you request.
We accept most major commercial plans, Medicare and Medicare Advantage, and workers’ compensation. We are not currently accepting Medicaid. Many plans do not require a referral; HMO plans usually do.
Bring your card to the first visit. Procedures usually need prior authorization, which our team handles.
Insurance & billing →Fax the referral and recent notes to (832) 916-2480. Your patient is contacted within one business day and you receive a consult note after the first visit.
Refer a patient →Ask to be added to the Pearland list; we will call when scheduling opens. Until then Pearland patients are welcome in Houston, about 15 miles away.
Pearland details →If you are having a medical emergency — new weakness, loss of bladder or bowel control, chest pain, or pain after a serious injury — call 911 or go to the nearest emergency room instead of requesting an appointment.
