Practice policies.

Clear expectations help every visit run on time and keep your care safe. Here are our policies on scheduling, medications, records, forms and billing.

In short

Please give 24 hours’ notice for cancellations. Controlled medications are prescribed under a treatment agreement with monitoring. Refills need 3 business days; forms need 5–7. Copays are due at check-in.

Appointments and cancellations

  • Please give at least 24 hours’ notice to cancel or reschedule an office visit or procedure.
  • Procedures require authorization, staff and equipment time; late cancellations and no-shows may result in a fee and, if repeated, discharge from the practice.
  • Arrive 20 minutes early if you have paperwork to complete; arrivals more than 15 minutes late may be rescheduled.

Controlled-substance policy

When opioids or other controlled medications are part of a plan, they are prescribed under a written treatment agreement and monitored — including Texas Prescription Monitoring Program checks and periodic urine drug testing — for your safety and as required by Texas law. Prescriptions come from one physician and one pharmacy; lost, stolen or early refills are not replaced. Read more about how we approach medication management.

Refills

  • Request refills through the portal or by phone at least 3 business days before you run out.
  • Refills are not issued after hours or on weekends.
  • A visit may be required before a refill if you are overdue for follow-up.

Medical records and forms

  • Records are released with your signed authorization; a fee may apply for personal copies as allowed by Texas law.
  • FMLA, disability, work-status and other forms take 5–7 business days.
  • Imaging performed elsewhere must be requested from that facility.

Financial policy

  • Copays are due at check-in. Balances after insurance are billed to you; payment plans are available.
  • We verify benefits as a courtesy, but your plan’s rules govern coverage. Prior authorization is not a guarantee of payment.

Respectful environment

We are committed to a safe, respectful environment for patients and staff. Abusive language, threats or harassment may result in discharge from the practice. Please leave firearms and weapons at home; Texas Penal Code notices are posted at each office.

Your health information is protected under HIPAA. Our privacy policy explains how the website and the practice handle your information and how to request a copy of our Notice of Privacy Practices.

Frequently asked questions

What if I am running late?

Call us. Arrivals more than 15 minutes late may need to be rescheduled so other patients are not delayed.

How do I request a refill?

Through the patient portal or by calling during business hours, at least 3 business days before you run out. Controlled-substance prescriptions are not refilled after hours, on weekends, or early.

How do I get my records?

Sign a records-release form (available at the office or through the portal). Texas law allows up to 15 business days; most requests are completed sooner. Records sent to another physician for continuing care are free of charge.

Do you complete FMLA, disability and work forms?

Yes, for established patients. Allow 5–7 business days. A form-completion fee may apply for forms not covered by insurance.
